    Insulin Efsitora Alfa: The Once-Weekly Basal Insulin Fc-Fusion Heading for an FDA Decision (July 7, 2026)

    The problem: a century of daily basal insulin

    For people who need it, basal insulin is the background layer that keeps blood glucose steady between meals and overnight. The catch is that even the best modern long-acting insulins - glargine (Lantus, Basaglar) and degludec (Tresiba) - are still daily injections. That is 365 shots a year, every year, and the friction is real: injection burden and dosing fatigue are among the reasons many people delay starting insulin or drift off it once they do. The obvious question has hung over the field for years: could you deliver a whole week of basal coverage in a single, well-behaved injection?

    What efsitora is: insulin bolted to an antibody tail

    Insulin efsitora alfa (Eli Lilly's LY3209590, originally called 'basal insulin Fc' or BIF) is that attempt. It is a fusion protein. Take a novel single-chain version of the insulin molecule and connect it, through a short peptide linker, to the Fc region - the stem - of a human IgG2 antibody. Insulin on its own is cleared in hours; the antibody Fc changes everything. It is bulky enough to slow how fast the drug is absorbed and eliminated, and, crucially, it plugs into the neonatal Fc receptor (FcRn), the same recycling pathway that lets antibodies circulate for weeks. Instead of being broken down, efsitora gets salvaged and sent back into circulation. The payoff is a half-life of roughly 15 to 17 days and an unusually flat profile: the peak-to-trough ratio is about 1.14, meaning insulin levels barely fluctuate across the week. One injection, seven days of steady basal insulin.

    How it works: it's still just insulin

    It is worth being clear that efsitora's mechanism at the cell is not exotic. The insulin portion binds and activates the insulin receptor exactly as any insulin does - pushing glucose into muscle and fat and telling the liver to stop overproducing glucose. The innovation is entirely in the pharmacokinetics: the Fc-fusion is a half-life-extension trick, not a new way to lower glucose. That is why efsitora is best understood as a delivery-cadence advance - turning a daily hormone into a weekly biologic - rather than a more potent glucose-lowering drug. The same Fc/FcRn playbook is now being used across the industry to stretch other short-lived hormones into monthly or weekly dosing.

    What the QWINT trials showed in type 2 diabetes

    Efsitora was tested in the Phase 3 QWINT program - five parallel trials. The type 2 diabetes results were consistently non-inferior to daily insulin. In QWINT-1, 796 insulin-naive adults were randomized to weekly efsitora (using a simplified algorithm of just four fixed doses) or daily glargine over 52 weeks: A1C fell 1.31% with efsitora versus 1.27% with glargine. The standout wasn't the A1C - it was the titration. About three-quarters of patients stayed on one of four fixed doses, the median number of dose adjustments was 2 with efsitora versus 8 with glargine, and clinically significant or severe hypoglycemia was actually numerically lower (roughly 0.50 versus 0.88 events per participant-year). QWINT-2 (vs degludec) showed A1C drops of 1.34% vs 1.26%; QWINT-3 in insulin-experienced patients showed 0.86% vs 0.75%; and QWINT-4 was a dead heat at 1.07% each. The story in type 2 diabetes: same glucose control, far fewer injections, and a simpler dosing scheme.

    The type 1 caveat: QWINT-5 and hypoglycemia

    Type 1 diabetes is a harder test, because those patients have no insulin of their own and less margin for error. In QWINT-5, weekly efsitora (with mealtime lispro) was non-inferior to daily degludec on A1C - the two were essentially tied, with a treatment difference around 0.05%. But the safety signal matters: combined level 2/3 hypoglycemia was higher with efsitora (rate ratio about 1.21, p=0.016), and severe hypoglycemia occurred in about 10% of the efsitora group versus 3% on degludec. The investigators themselves flagged that the way a weekly insulin is initiated and titrated in type 1 diabetes needs more work. This is the honest asterisk on efsitora: an ultra-long-acting depot is convenient, but if a dose is too high you cannot pull it back quickly, and in the brittle type 1 setting that showed up as more low-glucose events. Notably, Lilly's current FDA submission is for type 2 diabetes, not type 1.

    Why it matters now: an FDA decision and a two-horse race

    As of mid-2026, insulin efsitora alfa is under FDA review for type 2 diabetes, with a decision expected in the third quarter of 2026. If it clears, it will not be alone: Novo Nordisk's once-weekly insulin icodec (Awiqli) is already on the board, so efsitora would step into a genuine two-horse race to define what weekly basal insulin looks like in practice. The bigger significance is conceptual. Efsitora is one of the clearest demonstrations that antibody-Fc engineering can convert a fast-cleared peptide hormone into a once-weekly product - the same idea driving long-acting metabolic peptides like the monthly GLP-1 candidate MET-097i. The caveats are specific and worth repeating: efsitora matches, rather than beats, daily insulin on A1C, so its case rests on convenience and simpler titration; it is investigational and not yet approved; and the type 1 hypoglycemia data are a real limitation.
